Dosage and Administration of Tramdol

Tramadol is intended for use by adults (those 17 years of age and older). Patients with moderate to moderately severe chronic pain who do not require immediate relief should start with a low dose of Tramadol and increase the dose until the lowest effective dose is reached. The recommended starting dose is 50 mg per day, which can be increased by 50 mg every three days until a dose of 200 mg is reached. After that, Tramadol hydrochloride tablets can be taken in amounts of 50 mg to 100 mg every four to six hours as needed. The total dose should not exceed 400 mg in any one day.

Patients who need immediate pain relief can determine with their doctors if the benefits of starting with a higher dose outweigh the risks of doing so. In some cases, patients who start with a high initial dose end up discontinuing use of Tramadol due to adverse effects. If the benefits of a higher initial dose outweigh the risk, a patient seeking immediate pain relief can take 50 to 100 mg of Tramadol hydrochloride every four to six hours. The dosage should not exceed a total of 400 mg in one day.

 

Individualization of Dose

When managing pain with medication, it is important to use the lowest possible effective dose. When patients begin using small doses of Tramadol and increase the dose slowly upward, there are fewer cases of adverse side effects that cause patients to discontinue use of the drug. It is easier to tolerate a medication over time when the body is allowed to adjust to it slowly.

  • Patients with a creatinine clearance that is less than 30 mL/minute should increase the interval between doses to 12 hours. The maximum daily dose for these patients should not exceed 200 mg.
  • Since only 7% of administered Tramadol is removed in the process of hemodialysis, patients undergoing that procedure can take regular doses of Tramadol
  • Patients with cirrhosis should take a maximum dose of 50 mg of Tramadol every 12 hours.
  • Elderly patients should use caution and start with a low dose when taking Tramadol. This is due to the fact that older patients are more likely to have liver, kidney or heart problems and are also more likely to be taking other medications. Patients older than 75 should take a maximum dose of 300 mg of Tramadol per day.
